1. Conirm your discount eligibility by uploading or faxing proof of your active membership status.

Submit a copy of one of the following items that clearly shows your name and the name of your membership organization and any

applicable dates. Write your Sprint account number and Sprint wireless number on each page you upload or fax to Sprint.

Participation in the Sprint Discount Program is subject to continued veriication under your membership organization’s Service Agreement with Sprint. Failure to demonstrate eligibility for your

membership organization may result in charges for beneits you are not eligible to receive. Discounts are subject to change according to company’s agreement with Sprint. Discount currently

only applies to data buy-ups/add-ons for Unlimited, My Way and Framily plans, Talk 450 and primary line on Talk Share 700. Discount may also apply to select previously available service plans/

add-ons.

Account Information

Once we validate your information, the discount will appear on your invoice in 1 to 2 billing cycles. If you provided a valid email address you will receive a conirmation email from us.

You will not be considered a iliated with your organization's account or Service Agreement and will not receive any service plan discounts until Sprint receives and validates your Sprint

Discount Program Eligibility Veriication Form and proof of eligibility. Please direct questions about your discount eligibility to your membership organization. Sprint is unable to issue

retroactive credits.

3. Check your account status at sprint.com/verify after 3 days.
